I prefer cash or checks, but also accept credit cards, Venmo or PayPal payments.
Personalized Gift Certificates are available upon request. Once you provide me with the exact name of the benefactor and recipient, address, and a short message (one-liner), I can send you the Gift Certificate via email or regular mail. Gift Certificates never expire and are valid indefinitely.
I don’t take insurance. However, if you have a FSA account, I can provide you with a receipt for reimbursement. Also, if you have a prescription from a doctor, you can usually get reimbursements from your health insurance.
Please download these 3 forms and fill them out before your appointment. You can either email them to me or bring them with you to your first appointment:
General Intake Sheet
Description of Complaints
I require at least 24 hours notice for cancellations. This allows me to contact someone from my waiting list, who would like to take the appointment. If you are unable to give me a 24-hours advance notice, I will need to charge the full amount of the appointment.
Since I work from my home office and I don’t have a waiting room, I ask clients to arrive at exactly the time of the appointment or no more than 5 minutes before, since I might still be with a client. I have a bench outside my courtyard; please wait there in case you arrive early.
